Juvenile Risk Reduction Case Management Program

The Juvenile Risk Reduction Case Management (JRRCM) program serves youth who have been adjudicated of or have admitted to committing an offense and who are assessed as moderate to high risk of committing a new offense. Youth are referred to the program by their Juvenile Community Corrections Officer (JCCO).

Through the use of comprehensive Case Management services, this program identifies and works with both the positive (protective factors and strengths) and negative (risks and needs) factors along with the ecological factors of family, peers, school, neighborhood and community. The JRRCM program is characterized by a cognitive-behavioral, pro-social modeling approach that includes the following:

•  Identification of risk areas to be addressed

•  Development of an individualized, strength-based case plan. Each plan will build upon natural supports in the community, particularly family supports

•  Coordination and facilitation of team meetings for the purpose of developing or revising case plans and assessing progress towards achievement of the individual service plan

•  Modeling of pro-social behaviors, use of motivational techniques designed to increase youths' motivation to change, and opportunities to coach and assist youth in developing pro-social responses and behaviors

•  Referral of youth and their families to needed treatment services designed to address assessed needs, such services incorporate evidence-based best practices

•  Monitoring the provision of services to assure provided services support evidence-based best practices, as well as effect on youth and their families

•  Regular review of youths' status with Juvenile Community Corrections Officer.

Each youth referred to the program has an individual service plan (ISP) created that builds upon the youth's strengths and natural community supports, while specifically addressing priority risk factors and needs. Development of the ISP is overseen by a comprehensive team comprised of: the youth, his/her JCCO, a Spurwink Regional Lead Clinician, Spurwink Risk Reduction Case Manager, youth's parents or guardian, youth's existing service providers and natural support persons (e.g., teacher, mentor).

The Spurwink Risk Reduction Case Manager facilitates realization of the youth's ISP by brokering or coordinating needed services, maintaining an ongoing relationship and contact with the youth, using the youth's natural supports, monitoring progress towards achievement of the ISP, and modeling pro-social behaviors.
