High Fidelity Wraparound Services
The goal of this integrated system of care is to “wrap” formal and informal services and supports around children and their families, helping them achieve important outcomes while they remain in their homes and communities. A Wraparound Facilitator engages families in the process utilizing partners to provide direct support, modeling and education for families and youth. As a team- and community-based practice model, High Fidelity Wraparound Services emphasize collaboration, family voice and choice, individualization, family strengths, cultural competence, natural supports, and persistence in sticking with families, even in tough times. Services are provided in homes, schools and multiple other locations throughout the community.
A Community Collaborative Board (CCB) oversees Spurwink’s High Fidelity Wraparound Services. The CCB is made up of a parent, youth, natural supports and representatives from multiple community sectors, including child welfare, children’s behavioral health, corrections, education, mental health, labor, law enforcement, health care, recreation, faith-based, family and youth organizations. This board meets monthly to establish and implement policies and procedures that support service integration for families with complex needs and adherence to the Wraparound model.
