Case Management

With recognition and awareness that supporting the family is integral to the child’s healthy development and well-being, case management services are family-centered and strength-based.

This program serves youth ages 0 – 20 years who have a diagnosis of one or more of the following:

• Mental retardation
• Autism
• Pervasive Developmental Disorder
• Developmental Delay
• Emotional Disturbance
• Behavioral Disorder
• Mental Illness
• Asperger’s Syndrome

• Case coordination
• Family support
• Strengths and needs assessment
• Individual service plan coordination
• Psychosocial assessment
• Advocacy
• Locating and accessing appropriate services and resources
• Referrals
